ZIP CODE: The zip code to which the CC bank statement is mailed.

CREDIT CARD NUMBER: List all digits

EXPIRATION DATE: example: 07/14

CVV CODE: This is the Credit card Verification Value which is the last three digits on the back of the Visa or Mastercard, or for AmEx, the four digits on the right-side face of the card
